FormSherpa Referral Dashboard Overview
- Navigate to the Referral Dashboard by selecting “Referral Dashboard” on the homepage.
- The dashboard has three main sections:
- Referral Summary Cards – counters at the top showing your referral count by category (Active, Completed, Pending, etc.)
- Assigned Referrals Table – shows all referrals assigned to you.
- Submitted Referrals Table – shows the referrals you’ve sent to other members.
- The New Referral Form at the bottom of the page allows you to submit new referrals.
Update Referral Status & Send Referrals
- To accept a referral click on the green “Accept” button. Once accepted the referrals phone number will populate.
- To decline a referral click the red “Decline” button. You will not see the referral information if the referral is declined.
- Once you’ve completed an initial session/appointment you can click the blue “Complete” button.
- When you complete or decline a referral you will have the option to archive the referral so it is no longer in your assigned referral table. These referrals are visible in the “Archived Referrals” table.
- A new referral can be submitted by completing the fields in the “Submit New Referral” Form.
Send External Referrals
- Navigate to the “Submit New Referral” Form below the “Archived Referrals” table.
- Enter patient/client name and phone number.
- In the “Recipient Type” field, select “External Recipient”.
- Fill out the “Recipient Email” and “Recipient Name” fields. An invitation email with instructions on how to create an account will be sent to the recipients email. Once registered, the recipient will be able to view and update the referral status.
Register for a Community Event
- Navigate to the “Collaboration Dashboard” page location on the main menu.
- Select an event. Once selected you can view event details including location, spots available, organizer contact information, date/time, etc.
- At the bottom of the page you have the option to register for the event. Once registered, you will receive a confirmation email, verifying that you’ve signed up.
- If you aren’t ready to register for an event, you can save an event and complete registration in the future.
- To cancel an event, click the “Cancel Registration” button.
Search Member Directory
- Navigate to “Members” on the main menu.
- Filter the member directory by specialty such as “crossfit” or “dietitian”.
- Select a profile to learn more about the user.
- User profiles contain relevant information such as contact information, location, and website.
